About Macrotia

The appearance of the ears plays an important role in overall facial harmony. When the ears are disproportionately large, they can draw unwanted attention and create a sense of imbalance. A formal diagnosis of macrotia involves a physician measuring the ears and comparing the results with normal ear measurements. While the exact causes of macrotia are unknown, it is categorized as a congenital condition, meaning it is present from birth. It is usually apparent early in life, as the ears reach near-adult size by age five or six. At this stage, parents of patients with macrotia may consider early intervention. (2)

About Ear Reduction

Ear reduction surgery is a refined art, requiring surgeons to carefully remove portions of cartilage and tissue to create ears that appear more harmonious with your features.

Dr. Sherman performs ear reduction procedures at our in-house ambulatory surgery center. He typically uses local anesthesia; however, sedation anesthesia may also be administered by a board-certified anesthesiologist if necessary. The duration of the procedure can vary depending on the extent of the corrections being made, but most ear reductions range from 2-3 hours.

After preparing the targeted areas, Dr. Sherman will make an incision along the inside of the helix, or the curved outer edge of the upper ear. This incision may be combined with an incision on the back of the ear, depending on the concerns you are addressing.

Dr. Sherman will then remove tissue and cartilage from the ears to reduce their size. He will apply supportive sutures, close your incisions, and wrap your ears in protective bandages. You will also be given a compressive headband to wear during the initial stages of your recovery.

Candidates

Suitable candidates for ear reduction include patients with abnormally large ears that are disproportionate to their features. For minors, the procedure is generally considered once the outer ear (auricle or pinna) has reached 80–90% of its adult size, typically by ages 5 or 6. (2)

All patients under the age of 18 will require parental consent in order to undergo the procedure. Dr. Sherman will determine each patient’s eligibility for ear reduction during their consultation.

Personal Consultation

Before undergoing an ear reduction for macrotia, you will attend a personal consultation with Dr. Sherman. He will explain the procedure in depth, take the time to understand your goals, and discuss your medical history. He will also evaluate your ears and overall facial proportions.

If you are ready to schedule your consultation, please submit our online form or call our office directly at (212) 535-2300.

Preparation

Dr. Sherman will provide you with a set of personalized preoperative instructions to help you prepare for your ear reduction procedure.

Patients may be advised to:

  • Discontinue certain medications or supplements that have blood-thinning properties with approval from a primary care physician
  • Schedule time away from work or school—typically one week
  • Arrange transportation home
  • Cease nicotine use several weeks in advance

Recovery

During your recovery, you may experience swelling, bruising, or tenderness around the ears. These effects are temporary and typically dissipate on their own, but may also be managed by following aftercare instructions and using pain medications prescribed by Dr. Sherman. It is recommended to rest with your head elevated to help minimize any further swelling.

After five to seven days, you will need to return to the office for a follow-up appointment so that Dr. Sherman can remove your headband. After approximately ten days, he will remove the sutures as well.

Most patients can return to their routines after one week, but they will need to refrain from high-impact or physical activities until they are cleared by Dr. Sherman.

Results

After your procedure, you will be able to notice an immediate difference in the shape and size of your ears. The final results of your procedure will become more apparent after the bandages are removed and any remaining swelling subsides. You will be able to see smaller, more proportionate ears that you can feel more confident about. Many patients report feeling empowered to finally wear hairstyles that expose their ears.

The results of ear reduction are permanent, although the ears will continue to be affected by the natural processes of aging over time.

Corresponding Procedures

Ear Pinback

Some patients have ears that stick out in addition to being larger than average. If you share both of these concerns, Dr. Sherman can combine an ear reduction procedure with an ear pinback procedure. This procedure helps the ears lie flatter against the head.

FAQ

How long does it take to perform an ear reduction surgery?

The exact duration of the procedure can vary depending on the complexity of the case and whether one or both ears are being treated, but it typically takes between two to three hours to perform.

What type of anesthesia is used when performing an ear reduction for macrotia?

We typically perform this procedure using only local anesthesia for adult patients, which allows for a comfortable yet conscious experience during surgery. Sedation can also be used if necessary. For younger individuals, general anesthesia is generally recommended to ensure safety and minimize movement during the procedure.

Are there any risks associated with ear reduction surgery?

As with any surgical procedure, there are certain risks involved. These risks include infection, bleeding, asymmetry, scarring, changes in skin sensation, or dissatisfactory aesthetic results. While serious complications are uncommon, Dr. Sherman will thoroughly discuss all potential risks and benefits with you during your personal consultation, ensuring you feel confident and well-informed before moving forward. Choosing an experienced ear surgeon minimizes risks.

When can I see the results of my ear reduction?

After your procedure, an immediate change in ear size will be apparent. However, your ears will be covered in bandages, and initial swelling may obscure final results. After Dr. Sherman removes your bandages and headband, you will be better able to see the results of your procedure. Final results become more refined and visible over the course of several months as your ears continue to heal.

Can my ear reduction for macrotia be combined with another procedure?

Yes, your ear reduction procedure may be combined with another ear correction procedure for more comprehensive results. During your consultation, Dr. Sherman can provide you with tailored suggestions based on your aesthetic goals.

How soon after ear reduction can I return to work or school?

Most patients can return to non-strenuous work or school within a week, as long as they take certain precautions. More physically demanding activities should be postponed for several weeks, following Dr. Sherman’s guidance.

What is the recovery process like during the first week after ear reduction surgery?

During the first week, patients can expect mild discomfort, swelling, and a feeling of tightness around the ears. You will wear a protective headband and bandages to support healing and maintain the new ear shape. It is important to keep the area clean, avoid sleeping on the ears, and minimize any activity that could cause bending or pressure. Dr. Sherman will provide detailed aftercare instructions to ensure a smooth recovery; following these instructions is important to ensure an optimal experience.
